We providing Personalized Care to Clients such as intensive Support Services. Here is an outlined as follow:
Individualized home supports: Services for people who live in their own home or family home that provide support and/or training in community living service categories. There are three types of individualized home supports:
• Without training
• With training
• With family training.
All types of individualized home supports can be provided in the person’s own home, family’s home or in community spaces used by the general public, and either in person or remotely.
Support in community living service categories: Cueing, skill maintenance, guidance, instruction, assistance with activities of daily living, assistance with coordination of community living activities or direct supervision. Support must be within an allowable community living service category.
Training in community living service categories: Skill-building and instructional services to acquire, retain and improve the person’s experience living in the community. Training must be within an allowable community living service category, and training must meet identified needs specified in the person’s assessment (e.g., MnCHOICES, long-term care consultation).
Adult companion services: Non-medical care, supervision and socialization provided to a person age 18 or older.
Independent living skills (ILS) therapies: Art, music or recreational therapies with specific therapeutic goals and outcomes.
Respite Care: Short-term care services provided to a person when the person’s primary caregiver is absent or needs relief.
Primary caregiver(s): Person or people principally responsible for the care and supervision of the person who receives services.
In-home respite: Respite provided in the person’s home or place of residence.
Out-of-home respite: Respite provided outside of the person’s home or place of residence.
Night supervision services: Overnight assistance and supervision provided by an awake staff in a person’s own home. Own home: For the purposes of this page, a person’s own home means a setting the person owns, rents or leases that is not operated, owned or leased by a provider of services or supports.
